A participant form must be completed for all visitors to JAM Center. If you plan on bringing minor children into our facility, please ensure that a parent or legal guardian has completed the form below for the child prior to arrival.

By adding a minor child to the form below, I acknowledge that I am the custodial parent or legal guardian of the child and authorized to sign on their behalf. All individuals listed on a single form must reside in the same household. Separate forms are required for minors outside of my custody or that live at a separate address, and participant forms for minors must be signed by the parent or legal guardian of the minor for entry to the JAM facility.
